General Certificate of Conformity (GCC)

Instructions on filling out
the General Certificate of Conformity form

General Certificate of Conformity must be filled out in English
and electronically (not manually/handwritten)

  1. Describe the product in enough detail to allow a match to the product.
  2. The NMG Purchase Order number.
  3. The Vendor Style number.
  4. Provide the date(s) when the product was manufactured by month and year.
  5. Actual factory location is street address, city and county where finally manufactured or assembled. 
  6. Full name, mailing address, telephone number and email address of the foreign or domestic manufacturer.
  7. The plant manager responsible to assure compliance with (retailer/vendor/buyers) requirements.
  8. Name, full mailing address and telephone number of the US importer.
  9. Testing lab information.  Identify the name, address, telephone number of the testing lab.
  10. Provide Garment Test Report dates and numbers for all colors of product represented in the shipment. If Garment Test Reports are not available, provide Fabric Test Report dates and numbers.


    The certification must be based on a test of the product or a reasonable testing program.


    For all apparel products a certification is required. For many apparel products there will be a test.  For some apparel products, the fiber is exempt from testing. If the product is exempt from Flammable Fabrics Act testing, indicate by checking the box. This exemption applies only to garments made exclusively from acrylic, modacrylic, nylon, olefin, polyester, wool or any blends of these specifics.  Also, any plain surface fabric weighing 2.6 oz per sq. yd. or more is exempt from testing.

  11. Provide a soft copy of all required test reports related to children’s toys and child care articles that are intended for use by children under 12 years of age.

    Both toys and children’s articles (which includes footwear, apparel, jewelry and home items) will have to be certified based on third-party testing of the product by accredited third-party laboratories.  A list of laboratories that are accredited by country can be found at the following CPSC web link:   http://www.cpsc.gov/cgi-bin/labapplist.aspx

  12. Provide a soft copy of formaldehyde emissions test reports related to the CARB Act for Medium Density Fiberboard (MDF), Hardwood Plywood (HWPW), and Particleboard (PB).
  13. Mark the box indicating the rules, bans, regulations, and standards applicable to the product.
